Getting ready

Before the visit, you’ll receive a reminder with notes for how to get ready for the visit. Also, the day before the appointment, the health care practitioner may call you to introduce themselves and answer any questions you have about the visit.

Notes for preparing
 

  • Wear shoes that can be easily removed
  • Have any appointment cards for doctor and specialists ready
  • Make sure all medications are in their original bottles and are ready to review
  • Think about what questions or concerns you’d like to discuss and, if helpful, make a list

During the HouseCalls visit

You’ll have 45 to 60 minutes with the health care practitioner for a physical, health screenings and more. A HouseCalls visit can be completed while sitting at the kitchen table or in the living room, and you can use the time to ask whatever health-related questions you may have. The visit is driven by you and your needs, so screenings and conversation topics can vary.

After the appointment

At the end of the visit, you’ll receive a personalized checklist of items to discuss with your regular doctor. Also, a summary of the visit will be mailed to you and your doctor.

A member must complete a HouseCalls visit within the 2024 calendar year to be eligible for the $15 reward. Please allow 1 week after the completion of your HouseCalls visit for the reward to be loaded to your UCard. Use your reward on approved items at a participating retailer near you, including nationwide chains and neighborhood stores. If you have questions about your UCard, visit the UCard Hub on your plan website or call the number on your UCard. By using the reward funds on your UCard, you agree to the Terms and Conditions for Rewards Funds Accessible with S3 Cards, which are available at ucard.uhc.com.
